The Annual Membership Application form can be downloaded below.

The form fields can be filled in using Adobe Acrobat Reader DC.

Please fill out the required spaces and mail to the listed address with a check made payable to Sportcast USA. Those enrollments that are mailed in may be pre-approved by the Sportcast USA Board of Directors.

Alternatively, it can be filled out prior to your first event of the casting season and brought to the event. At this time the approval is subject to the approval of a board member or person designated by the Sportcast Board Of Directors.

Sportcast USA reserves the right to deny membership.
